Eligibility Requirements:

Must be 18 years of age on or by January 10, 2026
All participants must submit waiver & Medical Clearance Form
Non-refundable registration

Registration Categories:

Early Bird Registration (ends 12/1): $70
General Registration: $85
Under 21 Rate (must be under 21 by 1/11/26 and show ID): $40
Includes all tryout events, Phoenix frisbee*, & 1 month of watchUFA.tv**

Arrive promptly at 4:40 PM to sign in and receive your pinnie/number. Top performing players will advance to invitational tryout rounds tentatively scheduled for the last two weekends in January at an outdoor location TBD.

* Must be picked up in-person at tryout event | ** For first-time subscribers only, must redeem by April 15.

MEDICAL CLEARANCE FORM - REQUIRED

General Medical Clearance Form, completed and signed by participant’s primary care physician. This form can be emailed to your doctor’s office, though they may ask to complete the form in person if you haven’t had a physical examination in the past year.
